Air Force PJ Jason Sweet - Part Two | Mike Ritland Podcast Episode 149

About this video

Former PJ (Air Force Pararescue) Ritland’s guest recounts his deployment to Afghanistan conducting casualty evacuation missions. The guest describes multiple high-intensity missions, including a hot LZ extraction under fire (mission 0427 Alpha), emergency medical procedures performed in combat, a mass casualty event recovering deceased British soldiers, and a near-miss on his final mission. He shares technical details of combat medicine, the coordination between rescue assets, and reflects on the realities of war.

Key takeaways

• Combat casualty evacuation requires rapid decision-making under fire and coordinated deconfliction between multiple rescue assets (Guardian, Tricky, QRF)
• The guest describes advanced trauma care procedures used in combat including interosseous blood infusions, endotracheal intubation, and drug administration under austere conditions
• Mission 0427 Alpha involved extracting what was reported as a wounded soldier but was actually deceased; British forces may have misreported the casualty status to secure immediate evacuation during intense contact
• The guest experienced tunnel vision during his first firefight and credits Air Force Pararescue training with preparing him medically, though the gunfight itself was the critical learning experience
